Once filled, screw the bottom end knob clockwise to dispense toothpaste directly from toothbrush head. Screw in and suction fill your preferred toothpaste into the reservoir by twisting bottom end knob counter clockwise.

It has a toothpaste reservoir in the handle, toothbrush head with integrated dispenser, and cap that doubles as rinse cup. The original test toothbrush head has been discarded however I am including 4x brand new heads and 2x optional toothpaste universal nozzles.
